As a Community Nurse specialising in home-based care, you will bring vital healthcare services directly to patients in their homes, providing personalised and accessible medical support to those unable to attend hospitals or clinics. Working independently, you will manage a caseload of patients with diverse medical needs, from elderly individuals managing chronic conditions to patients requiring short-term post-operative care.
Your duties will include conducting assessments, developing and implementing personalised care plans, and administering medications and treatments such as wound care, injections, and IV therapy. You’ll also monitor patients' progress, adjusting care plans as necessary to optimise health outcomes.
